2. I will require a short app completed and signed, with name of insured group, contact name, address, phone/fax numbers, dates of coverage, departure and arrival point, along with a type written or neatly printed list of NAMES AND BIRTHDATES of insureds. ( Emailed or scanned, emailed lists are appreciated).

One or more persons in your group should be responsible to collect the actual premium which can be remitted via Credit Card or cheque payable to Ray Battiston Insurance.

Any type of group provided all insureds have basic government health insurance (such as OHIP). I can also insure any Player, Coaching Staff, Family or Fan who are going on a sports tournament. Right up to age 70. For ages over 70, I have alternate plans available. Premium rates start at $1.70 per day per person to age 30;  Almost all sports activities can be covered, as well as personal activities. The day rate begins at 12:01 am and finishes at 11:59 pm of the last day of coverage. Policies must be purchased prior to travel!

1. Immediate Policy Issue, with one policy number and one claims number.

2. $2,000,000. Canadian limits per insured person, with no Deductible.

3. 24 Hour Emergency Claims Hot Line Number.

4. Lower premium than purchasing individual policies-especially short trips!

5. No Pre-Existing Conditions Exclusion right up to age 70 on short trips of 35 days or less. (some conditions still apply).

6. Changes can be made right up to departure.
